1. Fail to give attention to details or makes careless mistakes in schoolwork
2. Have difficulty sustaining attention in tasks or play activities
3. Seem not to listen when spoken to directly
4. Not follow through on instructions and fail to finish schoolwork or chores
5. Have difficulty organizing tasks and activities
6. Avoid, dislike, or be reluctant to engage in tasks that require sustained mental effort
7. Lose things necessary for tasks or activities (e.g., toys, school assignments, pencils)
8. Be easily distracted by extraneous stimuli
9. Be forgetful in daily activities
